Record audio onto a word doc??

Is it possible to record audio to a Word document? We have a Performance Evaluation sheet and there is a comments section for the supervisor to leave comments. I was wondering if there was a way to simply record a comment rather than typing it. And then be able to play it within that doc. So it would maybe look like this: Comments: (and then have a play or record button here) Can this be done? -- Insert thoughtful/witty/meaningful/poetic/funny/deep/rude/stupid/random/comment here. Be unique...ya know, like everyone else.
